The Cochrane District System of Care refers to our regional coordinated access system. In practical terms, the CD System of Care is a mechanism that allows various agencies to work together to best support people who are trying to secure and maintain housing.\u00a0 The CD System of Care will also provide a transparent, fair and streamlined approach to access to housing and services for people experiencing homelessness in our community. The CD System of Care is committed to eliminating chronic homelessness and building a District where everyone has a safe and affordable home in a neighbourhood they choose to live in.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

The Cochrane District System of Care adheres to the following Guiding Principles in all their activities. All Partner Agencies are expected to abide by these principles in their work with the System of Care.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

  1. ENSURING DIGNITY AND CHOICE: We respect and celebrate people\u2019s ability to decide their own recovery goals and choose how\/how not to participate with services.<\/li>
  2. RESPECTFUL AND NON-JUDGMENTAL APPROACH: We meet people where they are and provide service in a manner that respects diversity and choice.<\/li>
  3. GRASSROOTS AND COMMUNITY-BASED: We ensure services are developed with a bottom-up approach and strive to support community-led initiatives in the development and delivery of services.<\/li>
  4. ACCOUNTABILITY: We are cost-efficient, transparent, and sustainable. We hold ourselves and our partners accountable to providing high quality and accessible services.<\/li>
  5. COURAGE: We work to move new ideas forward for the betterment of the community. We address and stand against stigma and discrimination.<\/li>
  6. STEWARDSHIP: We protect the Cochrane District System of Care brand and practice accountability in all actions of the organization.<\/li>
  7. INTEGRITY: We make decisions with the best interest of the community and organization in mind.<\/li>
  8. COORDINATION: We work to bring diverse stakeholders together around shared challenges and common goals.<\/li>
  9. COLLABORATION: We maximize the impact of public and private resources in our community by establishing shared goals and service models.<\/li>
  10. PARTNERSHIP: We believe that the best path to accomplishing our vision is through partnership with public and private organizations and members of our community.<\/li>
  11. HONESTY: We believe that relationships with partners and guests need to be open, trusting, and frank.<\/li>
